Cryostorage Disposition Consents

You have accessed this information because you currently have tissue stored at San Diego Fertility Center, IVF Laboratory. You have many options available to you regarding your cryopreserved tissue, other than continuing to store. Please read all options thoroughly, complete your chosen consent form, and mail, fax or scan the form to the IVF Lab. Where there is more than one intended parent, all options require signatures of both intended parents. Please make only ONE selection, note that all options require notarization, and some may also require additional paperwork. SDFC physicians do not recommend discontinuing storage within one year of initial cryopreservation or if you are currently pregnant. You will not need to pay your upcoming yearly storage fee once you have returned a completed Disposition Consent. If you would like to pursue other options outside of those listed here, we would be happy to work with you to accomplish your wishes.

In addition, if you have changed your address, or experienced a change in the status of your relationship with the other intended parent (if applicable), such as separation, divorce or dissolution of a domestic partnership, please notify us by completing the Information Update Form, below.
