Transportation Forms

In order to provide excellent service for our patients who wish to ship frozen human tissue in or out of the San Diego Fertility Center IVF Laboratory, and to maintain the highest standard of compliance with the many governmental and professional regulatory agencies that oversee our industry, we provide these resources for shipping your precious frozen material. Shipping human tissue in and out of our clinic is a task we take seriously for your protection. As you consider transporting your frozen tissue to or from San Diego Fertility Center, please be aware that:

  • Shipping of reproductive tissue occurs frequently, but is not free of risk
  • You have your choice of shipping providers
  • SDFC recommends you choose a shipping provider that offers shipping insurance
  • You may elect to divide your tissue into two shipments as an additional precaution

Shipping Eggs, Sperm or Embryos* from another location to SDFC

*SDFC charges a fee of $1650.00 to receive embryos that were frozen at another facility. This does not include the cost of shipping that is determined by the company you contract with to ship your embryos.

  • Transport Consent and Contractual Agreement: Allows us to receive and store your frozen tissue and is to be completed and signed by you and returned to us at this address at your earliest convenience.

  • Request for Embryo-Gamete Information: Form is to be sent to the clinic currently storing your tissue. Due to privacy regulations, they will not release this information to us without previous authorization from you. We will need that form completed by the clinic storing your tissue and returned to us along with the requested documentation. Once we review that information, we can determine if shipping this tissue to our clinic is your best option. Your nurse coordinator will be able to assist you with FDA requirements, if applicable.

  • Steps to Transfer to SDFC: Document provides you with information to assist you in arranging for the transport of your tissue to SDFC, once we determine if it are to be shipped. We strongly recommend that you use a courier that specializes in transporting human tissue, and we have included a few companies with whom we have worked many times. You are, of course, free to contract with whomever you choose. While we do not recommend any particular shipping company, we are aware of only two companies that offer insurance against loss or damage for shipments; Cryoport/Cryostork and IVF Couriers. If you are interested in insuring your shipment, please contact the shipping companies directly for terms and pricing.

  • Release Consent: Allows us to release relevant cryopreservation and medical records, as well as your tissues for transport. Please review the consent, indicate all types of tissue you would like transported, sign and return to us by e-mail scan.

  • Steps to Transfer From SDFC: Provides information to help you arrange for shipping from SDFC. We strongly recommend that you use a courier that specializes in transporting human tissue, and we have included a few companies with whom we have worked many times. You are, of course, free to contract with whomever you choose. While we do not recommend any particular shipping company, we are aware of only two companies that offer insurance against loss or damage for shipments; Cryoport/Cryostork and IVF Couriers.
